关于焦作市示范性普通高中评估认定暨复验工作情况通报
各县（市）区教育局，市直各普通高中：
为加强我市示范性普通高中建设，确保示范性普通高中在课程改革和素质教育等方面起到示范和引领作用。根据《焦作市人民政府教育督导团办公室关于开展市级示范性普通高中评估认定暨复验工作的通知》（焦政教督办〔2009〕4号）要求，2009年10月20至28日，市政府教育督导团办公室组织部分市政府督学、教育专家及市教育局部分科室业务骨干组成评估组，依据《焦作市示范性普通高中评估细则（试行）》，遵循“基础达标与特色并重，突出特色与潜力”、“结果与过程并重，突出发展与提高”、“硬件建设与软件建设并重，突出管理和质量”的标准，本着“以评促建，以评促发展”的原则，就办学方向、学校管理、队伍建设、教育教学、办学条件、办学效益和示范作用等六个方面，对申请评估认定的三所普通高中和申请复验的六所示范性普通高中进行了认真的检查评估。现将有关情况通报如下：
一、基本情况
所评估学校认真贯彻党的教育方针，坚持正确的办学方向，以“办人民满意的教育”为宗旨，大力实施素质教育，不断提升教育教学质量，为区域经济社会发展做出了应有的贡献。申请复验的六所学校（市外国语中学、市十二中、修武一中分校、武陟中学、焦作四中、武陟城关高中）在被授予“焦作市示范性普通高中”称号以来，按照素质教育的要求，不断提升办学目标和教学理念，完善管理体系，改革教育教学，提高质量和内涵，充分发挥了示范引领和辐射带动作用。从评估结果来看，六所学校均达到优秀格次；申请评估认定的三所学校（焦作一中分校、孟州五中、焦作六中）近几年来不断改善办学条件，优化管理结构，突出特色办学，增强办学效益，教育教学工作均取得了长足的发展，呈现出了新的生机和活力。从评估结果来看，焦作一中分校、孟州五中两所学校均达到了优秀格次，焦作六中达到了良好格次。
二、主要成绩和优点
（一）以学生全面发展为宗旨，育人理念与时俱进，培养目标明确具体。
各个学校根据区域教育发展规划及自身实际定位办学方向和培养目标，不断创新育人理念，初步形成了符合素质教育要求的办学思想。武陟城关高中制定了“三年抢占制高点，五年形成一定特色，十年打造城关高中品牌”的奋斗目标，把“面向全体、以人为本、重视个体、发展个性”作为自己的办学宗旨；市十二中、外国语中学以“办人民满意的教育”为宗旨，以人文关怀和科学管理为抓手，坚持“特色立校”，并根据时代和社会发展的需要，不断赋予其新的内涵；武陟中学以专家治校为根本，以“为社会主义培养品德高尚、基础扎实、特长突出、习惯良好的合格人才”作为培养目标，大力倡导成功教育，积极扩大优质教育资源，稳定办学规模，为学生创造成功的机遇、建筑成功的平台，使学校呈现了良好的发展态势；焦作一中分校、修武一中分校、六中、四中等学校以学生的“成人”与“成才”为标准，注重培养学生的创新精神和实践能力，重视个体差异，做到因材施教，不偏向、不歧视任何一个学生，着力培养学生的自我管理、自主学习、独立生活能力和合作精神，为学生个性发展提供平台，为学生的幸福人生奠基。
（二）创新工作机制，优化管理结构，教育教学秩序井然。
一是坚持校长负责制，不断改革管理体制。各个学校积极实施校长负责制，领导班子配备合理，团结务实，作风民主。坚持政务公开制度和教代会制度，学校的人事变动、评优评先、晋级以及学校的收支情况能及时公示，各学校校产管理严格，学校维修制度健全。各处室既分工明确，又相互合作，形成了以校长为核心的校级决策机构和以处（室）、年级为基本单元的管理网络。二是不断创新工作机制，优化管理结构。为充分调动教职工的工作积极性，各学校均出台了班级量化考核办法、教师、行政人员评价办法、班主任评价制度和教师聘任制度等一系列行之有效的评价机制，各种规章制度健全，教职工工作积极性较高。例如修武一中分校继续推行教育教学小区管理模式和双向互动管理模式，着力构建富有特色的管理育人机制，保障了学校工作健康有序地发展；市十二中出台了《教师量化考核方案》，进一步探索和完善了科学评价机制；焦作一中分校坚持“寓管理与服务之中”的“管、理并重”的指导思想，通过建立健全教职工的评价、反馈、矫正机制，优化学生会、团支部等学生管理机构，基本使广大师生形成了自我激励、自我约束、自我教育、自我管理的运行机制；外国语中学先后制定了《绩效工资考核方案》、《教师五项评价体系》、《质量系数认定办法》，对每位教职工的德能勤绩进行定性、定量考核，充分调动了教职工工作积极性。各个学校各项制度健全，管理规范，教育教学秩序井然。
（三）深化教育教学改革，培植特色活力，教育教学质量持续提升。
一是教学常规管理严格规范。各个学校都十分重视教学常规管理，坚持教育教学工作面向全体学生，激发学生学习兴趣，积极调动学校、教师课堂教学的积极性，对教师的教案、学生作业以及互相听课，教育笔记和教学反思的检查、反馈等都能较认真地记录在案，教学常规落实普遍较好。二是加强新课程培训，改革课堂教学模式，打造高效课堂。各校都高度重视新课程改革工作，认真组织教师参加新课程及跟进式培训，培训合格率达到了100%。市十二中在新课改活动中以“解放自身智力因素，激活学生非智力因素，提升学生综合应用能力”为目标，引进新乡姚兆兵“两步六环高效大课堂”模式，结合阶梯式教学法，努力探索符合学生实际的教育教学模式；修武一中分校借鉴山东兖州一中“三步六段”高校课堂教学模式，通过课堂教学改革，基本形成了自己的“导学案式教学法”高效教学模式；焦作一中分校进一步提高国家课程的执行能力和校本课程开发能力，在高一、高二年级全面推行符合新课改理念的“循环大课堂”模式，成效显著。三是坚持校本开发，突出特色办学。市外国语中学有效利用校内外各种课程资源，突出外语特色，积极开展校本培训，形成了有特色的学校课程体系，编撰了《英语口语教程》、《演讲与口才》、《高三复习资料汇编》等校本教材；焦作一中分校按照“以校为本、特色为重、课题带动、规范创新”的校本教研方针，完善校本课程，形成了有校本特色的学校课程体系；十二中、焦作四中、孟州五中、武陟城关高中等发扬美术、体育等特色办学之路，结合本校实际开发校本科研，积极拓展学校艺术生的升学渠道，与省内外高校做好教学实践基地和生源供应基地建设工作，走出了“文化教育争奇斗艳，特长教育百花齐放”的特色之路，使特色教育成为学校素质教育的一张亮丽的名片。近三年来，各个学校毕业会考合格率、按时毕业率以及体育达标率等均在90%以上，大部分学校高考本科上线率稳中有升，艺体特长生升学人数和占升学总人数的比例逐年提高。绝大多数学校重视学生养成教育、心理教育和学生家庭教育，开展阳光体育活动，学生养成了良好的体育锻炼习惯，形成了健康的生活方式，毕业生体检合格率、学生体育合格率基本达到省定标准。
(四)改善办学条件，优化育人环境，精心营造文明和谐校园。
一是办学条件不断改善，办学水平逐步提高。武陟中学去年投入250多万元装备了2个多媒体教室，开通了校园网和校园广播网，新建了电子备课室，修建了塑胶跑道；孟州五中加大基础设施建设力度，使学校规模由原来的4规制达到了12规制，并投资建成了8个高标准的理化生实验室和2个理化生仪器室，学校图书达到两万余册，基本满足了教学需要；修武一中分校、市十二中投资兴建了运动场、规划了活动场地，购置体育器材，保证了学校体育教学工作和体育活动的正常开展；焦作六中去年投入50余万元建成了两个标准化的理、化实验室，更新了部分图书，修建了草坪、花圃，美化了校园环境；焦作四中近两年累计投资150万元，加大学校基建、教学设施、教师培训、校园文化的建设力度，保证了学校教育教学工作的持续健康发展；二是信息化水平得到提升。各学校坚持以教育信息技术现代化带动学校发展现代化，加强和改善电子监控系统和电视广播系统，装备多媒体教室，建设校园局域网和校长办公系统，数字化校园建设成效显著；三是突出德育的先导地位。市外国语中学充分发扬学校德育优势，积极构建“学校、家庭、社会”三结合德育网络，着力打造德育品牌；市十二中、四中进一步明确“德育为智育的保障”的教育思想，降低德育重心，将德育工作系列化、学龄化、目标化、具体化；修武一中分校构建了四维一体的德育网络，充分发挥四条主线的德育功能，使学生在良好的育人氛围中潜移默化地陶冶情操，激发创造潜能；四是重视校园文化建设，激发校园活力。各个学校利用名人名言、警句书壁、书韵长廊、激励学生的横幅标语等，努力营造奋发向上的校园文化，着力打造“书香校园”。大部分学校校园环境优美，整洁卫生，校园布局合理，建筑物规划整齐，有功能标志，生均绿化面积基本达到 1平方米以上。
(五)利用优质教育资源优势，加大对薄弱学校帮扶力度，积极发挥示范和引领作用。
各个学校充分发挥自身优势，合理利用教育资源，采取选派骨干、培训教师、结对帮扶、提供设备等形式对区域内薄弱学校进行帮扶，派出部分优秀教师到薄弱学校进行教学工作，加大兄弟学校相互学习和交流，对区域精神文明建设做出了一定的贡献。如市外国语中学、十二中、焦作四中等学校都认真安排城乡支教活动，加大对农村薄弱学校的扶持力度；孟州五中与沁阳四中、博爱二中等学校建立联谊学校，定期交流办学经验；修武一中分校与高村高中、武陟城关高中结成“互动联盟”，相互学习、优势互补、资源共享。
三、存在的问题
1.个别学校发展规划不科学、办学思想和培养目标不太明确。一是个别学校制定的五年或十年发展规划过于简单，不能体现学校的办学宗旨和办学特色，缺少实施方案和具体的措施；二是个别学校（焦作四中、焦作六中）制定的办学思想和培养目标大而笼统，没有与本校实际很好的结合。
2.部分学校的体育器材、图书仪器达不到规定标准。武陟城关高中受条件限制，生均占地面积及生均建筑面积太低，没有理化生实验室及相关仪器，体育器材不达标；孟州五中理化生仪器不达标；焦作六中的生物仪器已影响了教学；焦作一中分校基本上和总校在一起，根据两校的规模和现有的实验室个数比例，实验室不能满足两校的教学要求；焦作四中、武陟中学、焦作外国语中学阅览室管理不到位，无可供学生阅览的图书。
3.学校常规管理工作不到位。一是一些原始档案材料收集不够全面，档案管理不够规范；二是大部分学校在构建符合本校实际的校本培训体系工作方面重视不够，效果不明显；三是大部分学校没有专职的心理咨询教师，心理健康教育工作重视不够；四是个别学校的教代会不能经常召开，教学管理有待进一步加强。
4.个别学校校园文化氛围缺失。如孟州五中、外国语中学作为市级校园文化示范校，宿舍文化却极度匮乏；六中校园硬件和校园文化多年无变化，宿舍卫生差。
5.部分学校班额过大。
四、意见和建议
1.各个学校要按照素质教育和新课程改革的要求，不断提升办学目标和教学理念，提高质量和内涵发展水平。
2.进一步改善办学条件，优化育人环境。被授予焦作市示范性普通高中称号的学校须将每年所收择校费安排一定比例用于改善学校的办学条件（不含大型建筑项目），严格按照学校发展规划，不断完善和更新学校内部设施，确保学校的可持续健康发展。
3.不断优化教学管理，改革教育教学，扩大优质教育资源。一是进一步加强新课程改革力度，积极探索体现素质教育精神和符合校本实际的教育教学模式，打造高效课堂，构建有效作业，做好教学反思；二是充分发挥教科研的作用，严格教育教学管理，加强对符合本校实际的校本培训体系建设的指导，使校本培训在教改中更好的发挥作用；三是要进一步加强学校档案管理人员培训，进一步促进档案管理规范化。
4.加大对薄弱学校的扶持力度，进一步发挥示范引领和辐射带动作用。
